Collect departing employees’ reasons for leaving, workplace and manager feedback, and contact details. Use it for exit interviews and to spot ways to improve retention.
When to use it
- Before a scheduled exit interview
- When an employee resigns or finishes a contract
- To gather feedback from remote or departing staff
- To review recurring reasons employees leave
What it asks
- What is your name?
- What is your email address?
- What is the best phone number to reach you?
- What is your main reason for leaving?
- What worked well during your time here?
- What could we improve?
- What feedback would you like to share about your manager?
- Would you consider returning to work here?
- Is there anything else you would like to share?
Why it should be encrypted
Exit feedback can include personal contact details and candid comments about managers or workplace issues. Limit access to HR and the specific people responsible for reviewing the feedback.
Tips
- Make name and contact questions optional if you want to offer anonymous feedback.
- Tell employees who will review their answers and how the feedback will be used.
- Customize the questions to reflect your workplace and add a separate prompt for suggestions.
