Net Promoter Score Survey Template
Collect customer names, email addresses, a 0–10 recommendation score, and feedback on the score and ways to improve. Use it to understand customer satisfaction and follow up.
When to use it
- After a purchase or service interaction
- At regular intervals to track customer sentiment
- After a product update or new feature launch
- When following up with customers who give a low score
What it asks
- What is your name?
- What is your email address?
- How likely are you to recommend us to a friend or colleague?
- What is the main reason for your score?
- What could we do to raise your score?
Why it should be encrypted
Names, email addresses, and candid feedback can reveal customer opinions and business relationships. Keep responses readable only to the team responsible for customer feedback, not SealForm or unrelated staff.
Tips
- Explain how you’ll use responses and whether someone may follow up.
- Consider making name and email optional if you want more candid feedback.
- Use the same 0–10 question over time to compare scores consistently.
