Landlord–Tenant Dispute Intake Template
Collects contact details, rental and lease information, dispute details, notices, and documents from tenants or landlords seeking help with a rental dispute.
When to use it
- A tenant asks for help responding to a landlord
- A landlord seeks advice about a tenant dispute
- A housing clinic screens a new client
- A property manager records a dispute for review
What it asks
- Your name
- Your email address
- Your phone number
- Are you the tenant or the landlord?
- What is the rental property's address?
- When did the lease start?
- When did the lease end?
- Please describe the dispute.
- What notices have you received?
- Upload relevant documents
Why it should be encrypted
Dispute descriptions, contact details, notices, and lease documents can reveal private housing and legal matters. Only the intended adviser or review team should be able to read them—not SealForm or the other party unless the submitter chooses to share them.
Tips
- Tell respondents which documents are useful, such as notices or relevant lease pages.
- Ask them to leave out unrelated personal or financial details from uploads.
- Clarify who will review submissions and how respondents can share follow-up information.
