When to use it

  • Check in with students during the school year
  • Gather feedback after a wellbeing or anti-bullying initiative
  • Understand student concerns before planning support programs
  • Compare overall wellbeing trends across school terms

What it asks

  1. How safe do you feel at school?
  2. How stressed have you felt lately?
  3. How would you rate your sleep lately?
  4. Do you feel supported by adults at school?
  5. How often have you experienced bullying at school lately?
  6. Is there anything else you’d like us to know?

Why it should be encrypted

Answers about bullying, safety, and stress can be personal, and free-text comments may reveal who a student is. Encryption helps keep responses from being read by SealForm; share access only with staff who need the results.

Tips

  • Remind students not to include names or identifying details in comments.
  • Explain how responses will be reviewed and what support options students can use if they need help now.
  • Keep questions optional where appropriate, and share results in groups rather than singling out students.
